Ongoing Contract / Fixed Term Contract: One Year

I hereby apply to become a member of WE NETWORK music group mentorship.

1. Fees for music group mentorship programming is based on studio access.

Payment of $100 per month, for ongoing access, check in and accountability, will be due prior to the initial session for the current month by automated debit approximately on the same date each month.

Payments by Interac e-Transfer are sent to [email protected] (or by CanadaHelps alternatively)

2. Scheduling appointments can be made online or on the phone but are to be scheduled in advance.

3. Cancellations require 24 hours notice. Last minute cancellations cannot be rebooked the same week except with proof of an emergency situation.

4. Confidentiality is strictly adhered between both parties observed and practiced in the music mentor/ mentee relationship as well as coach / coaching client relationship.

5. Terms of service include obtaining consent before disclosing the confidential workings of the music mentorship / coaching relationship. Indeed, the membership keep information private about each other, and staff confidential at all times.
